changing template fields impossible

Description

It is not possible to remove template fields or change their properties if the the template is in use yet. This is reasonable, but applied too strict: even if you just created a template field and consequently the field is empty in all existing studies with the same template, you cannot remove or change anything concerning this new field because "the template is in use".

When I use a new field for the first time I sometimes want to change it a bit (name, characteristics)or after a while think it is better to remove it. That is not possible. It would be nice to have the ability to change a new template field when the fields are empty, then if you are the first one to create the field you can easily change things.

Because it is not possible to modify template fields that are in use, at the moment it is only possible to choose 2 sorts of diets in a diet treatment (low fat and high fat). If you want to add a diet via add/modify the event template you cannot modify the diet field (string list) because it is in use.

I've improved the template editor such that:

template fields that are used in a template can be edited if:
- the template field is never really filled in an entity AND
- the template field is not used in any other template (in that case the users of other templates might be confused because their template field is changed)

template fields that are used in a template can be deleted if:
- the template field is never really filled in an entity

The next step is to improve STRINGLIST and ONTOLOGYTERM fields, to make it possible to add list items or ontologies to choose from and remove list items and ontologies that are never used.

I've added the possibility to add new items to a stringlist field and remove items that have never been used. The same holds for ontologyterm fields. See changeset [1175]

Earlier changes can be found in changesets [1159] and [1160].

I've found another bug in the templateeditor in the ordering of fields. That is reported seperately in #202.

Please test whether the functionality works as expected.
