clarify sample generation / adjust column widths in study create/edit wizard

Description

In the study create/edit wizard, when the samples are generated, it's hard to read the sample name because the column is too small. Is it possible to make the name column bigger?
Also, it would make the sample generation more clear if besides the sample template, also the parent event group, sample and sampling event were displayed. Study.samples is a List, so the samples will be displayed in the order in which they are generated.

Use the following procedure to generate the samples in order:

For every EventGroup?

For every Subject in that group

For every SamplingEvent? in that group

Generate the sample

Then, display them grouped by their Sample template, but in the generated order (sort by id if neccessary).

Maybe it's an idea to add something like columnWidths="[name: 300px]" to the templateColumnHeaders tag?

So you get <wizard:templateColumnHeaders entity="${sample}" class="column" columnWidths="[name: 300px]" />

Cool! Added larger column width for sample names in [1146]. This works!

However, sample generation is not fully correct yet, but that is now described in #195, so closing this ticket.
